What is the transitional care model?

Roughly 20% of Medicare patients are readmitted to the hospital within 30 days of discharge, a costly and stressful event that the transitional care model (TCM) was designed to prevent. This evidence-based, nurse-led approach addresses the critical gap that often occurs when a vulnerable, chronically ill patient moves from a hospital to their home or another care setting.
